**Salary**: Between £45,000 - £55,000 (based on experience)

**Number of Post**: 2 Operational Leads

**Contract Type**: x1 permanent x1 maternity cover

We are recruiting two PCN Operational Leads (one role is permanent and the second role is to provide interim maternity cover). These two posts will join the team comprising one further PCN
Operational Lead and two PCN Network Officers. Each PCN Operational Lead will manage multiple work-streams driving the integration of health and social care within each of the 4 PCNs. All of these work-streams are multi-agency and therefore involve complex stakeholder management.

Our work supports the integration of healthcare services at a PCN level; ensuring that different health, social care, local government, education, patient group and other partners work coherently together to meet the specific needs of their local population. Our drive to create an integrated strategy and service delivery at a PCN level needs high quality programme management support, given the complexity and fast pace of our work.

Across Islington primary care, and with our wider North Central London partners, we are working to improve training and development for clinical and non-clinical staff, and establishing strong links across local organisational boundaries. We harness clinical leadership to shape our workforce and service improvement priorities, and connect primary care, local government, higher education institutions and others together to collaborate and deliver at-scale projects.

These post holders will work as part of a matrix structure in which they will report into the Chief
Operating Officer and working alongside the Training Hub, as part of the integration team. The role requires a project management approach that involves considerable responsibility for co-ordinating new models of care, development of new initiatives, embedding new workforce and other projects in accordance with stakeholder needs that are aligned to local, regional and national strategies.

The post-holder will require top class organisational talent, with leadership, stakeholder management and analytical skills. Applicants should have experience in successfully managing complex projects and change initiatives.

**About Islington GP Federation**

Islington GP Federation (IGPF) is a growing organisation representing 31 practices; we have established ourselves as a leader in new ways of working, including running Islington’s enhanced access primary care services (I: HUB) as well as supporting the Islington Primary Care Networks
(PCNs). Our current range of services includes I: HUB, Community ENT, the Gynaecology
Collaborative, practice-based pharmacy team comprising 43 pharmacists and a range of practice support mechanisms.

IGPF works very closely with a range of partners including the regional commissioning group, NHS
England, Healthy London Partnership, Public Health, local hospitals such as the Whittington Health and the London Borough of Islington.

IGPF is the host organisation for the Primary Care Network (PCN) workforce and the Islington
Training Hub, and has been working for over three years to create training and development programmes that meet the needs of staff working in primary and community care settings.

**Responsibilities**:
**Project Management**
- Work closely with the Clinical Directors, GP practices within each neighbourhood and PCN, to establish and develop capacity and capability for at scale delivery in primary care
- Be responsible for the effective design and delivery of key strategic PCN and integration projects which includes mobilisation of plans for new ways of working across primary care, using a population health management approach utilising HealtheIntent data.
- Facilitate GP practices, community services and other primary care stakeholders to work more closely together and to share skills and resources by facilitating development of new pathways and introducing innovation
- Be responsible for originating and then maintaining project plans, including project reporting, monthly highlight reports, updating the risk logs, producing exception reports and ensuring that all work remains on target to deliver agreed objectives.
- Ensure delivery of key elements against agreed milestones, including budget, timelines, outcomes measure and key performance indicators with demonstration of success.
- Develop appropriate policies and procedures in support of the project implementation.
- Contribute to developing a culture of support within the PCN including a culture of change that is both grounded and creative
- Act as a point of contact for PCN based services, ensuring these are well communicated and are effective. This will include liaison with key stakeholders and the fostering of local relationships
