This is an exciting opportunity for an outstanding Lead SEMH Practitioner to become part of our Alternative Provision transformation.

As a lead Local Authority in the DFE SEND and AP Change programme, we have the chance to advance educational support on offer for children and young people with Social, Emotional and Mental Health difficulties through a 3 Tier model. An opportunity has arisen for a newly created position of Lead SEMH Practitioner (Alternative Provision Specialist Taskforce APST) in the Hartlepool Borough Council Education Service. Initially the post will be based on a one year Fixed term Contract, or a secondment will be considered, with the possibility of extending.

As Lead SEMH Practitioner, you will provide highly engaging, relevant and innovative support to children and young people accessing provision through the APST.

**Tier 1**:

- Targeted support in mainstream schools (AP specialist early interventions and support to help at-risk pupils stay in mainstream school).

**Tier 2**:

- Time-limited placements (Short-term placements in AP schools to assess and address pupil’s needs, with the expectation of return to their mainstream school).

**Tier 3**:

- Transitional placements (Placements in AP schools for pupils who need support to move on to a new mainstream school or sustained post-16 destination).

The APST is a model based on early identification of need, providing intervention and support at the right place, at the right time and AP being used as a mechanism of intervention rather than a destination.

Tier 1 and 2, will provide Alternative Provision support for children in mainstream schools from Reception - Year 11. This post will input across Tier 1 and 2.

The post holder will have recent and current experience of supporting children and young people with Social, Emotional and Mental Health difficulties. They will have the ability to deliver learning opportunities to ensure children and young people make exceptional progress and are provided with the skills they need to be successful in their school career which will ultimately result in improved life chances.

You will work closely with Hartlepool’s schools to promote and support the best possible outcomes for vulnerable children through writing and delivering individual/ group plans, coaching, advising and delivering professional development to colleagues.

We are looking for someone who has demonstrated effective input in a school and has high aspirations and a passion for promoting the educational attainment of children with Social, Emotional and Mental Health difficulties.

The successful post holder will have experience of supporting vulnerable children in educational settings and will be able to demonstrate the impact of their work on improved educational outcomes.

The post holder will have an understanding of a wide range of Social, Emotional and Mental Health difficulties and will use their initiative and imagination to unblock significant barriers to the educational success for children and young people.

The post holder will be passionate about improving life chances for all young people and committed to supporting the needs of the Hartlepool child.
