To contribute to the effective daily management of clients referred to their caseload including prioritising clinical work and balancing other client related and professional duties. To provide clinical leadership to ensure where appropriate the service will work effectively alongside other services, to manage risk and prevent hospital admission for service users with the most complex and acute needs To work with the MDT, clinically reviewing and coordinating complex caseloads, delegating to treating clinicians and external agencies accordingly. To be able to work alone in the community in a variety of settings, independent of multi-disciplinary support whilst in direct contact with clients and their carers. To monitor, review and continually re-assess clients with complex care plans and objectives and facilitate modification of packages of care/targets as appropriate to enable achievement of their goals.
