**Job overview**:
An exciting opportunity has arisen for an enthusiastic, highly motivated, dynamic, experienced, and dedicated learning disability nurse to join our established innovative, community-based LD West Cheshire multi-disciplinary team

You will be joining a well-established and supportive team, where person centred care is at the forefront of everything that we do.

You will be responsible for delivering high-quality clinical care and supporting the clinical leadership within the community nursing team, ensuring all work is needs led, evidence based and in line with current legislation.

You will also support the leadership and development of services in line with the workstreams that are the focus for the care group, which includes, mental health, forensic, physical health, NDD and cognitive impairment.

CWP hosts CANDDID, the Centre for Autism Neurodevelopmental disorders and Intellectual Disabilities, which carries out research, education and training, and all clinicians are encouraged and supported to develop research and QI skills.

**Main duties of the job**:
The post holder will carry a caseload of clients with learning disabilities who have the most complex needs, where a very high level of clinical expertise is required to deliver care that will reduce health inequalities for people including behaviours of concern and/or autism.

The post holder will act as transformational role model/guide to deliver specialist learning disability assessment, intervention and person centred outcomes.

They will also be responsible for providing clinical supervision and appraisals of nurses within the team and supervise students while on placement with the team.
