The Home First service is an integrated clinical and care and support service that provides short term reablement intervention to enable people to maximise their independence and meet their health and social care needs within Midlands NHS Partnership Foundation Trust (MPFT).

The service seeks to offer rehabilitation programme for a time limited period to rebuild functional range of movement, confidence, relearn new skills by promoting people's independence through health & social care interventions. The Home First Integrated service operates from 7am to 10pm over 365 days per year.

The post holder will be responsible for working in close collaboration with the Home First Multi-disciplinary Team which includes Nurses, Therapists, Care Support Workers and Social Workers.

The post holder will have exemplary communication skills, enabling them to be a role model for others, as well as positively representing the Trust values, in the delivery of effective, efficient and high-quality services for service users. The post holder will be enthusiastic, passionate and a dynamic individual.

The shift patterns will be on a rotational basis over 7 days, so flexibility is required for this role.

The post holder will provide an effective and efficient administrative service to the North Home First Team

The needs of the service user will be the utmost priority. The principle of high-quality customer care is at the heart of this role.

Working within the service can at times be a stressful environment, by way of telephone calls, visitors and daily urgencies. The post holder requires the ability to work under pressure, with conflicting demands at times, show a willingness to assist others, be a key member of the team with a requirement to work flexibly in other administrative areas as appropriate as directed by their line manager.

1. Provide administrative support to the North Home First Team.
2. Act as the first point of contact for health and social care professionals.
3. Input onto data systems including Health Roster, Rio, Dashboards and any other systems required.
4. Provide a full range of office support systems including photocopying, distribution and archive filing (both hard copy and electronic), scanning and assisting other staff in the use of the Multi-Function Device (MFD).
5. Opening, sorting post and prioritising correspondence, preparing and collating documents.
6. Handle incoming and outgoing telephone calls in a polite and efficient manner. Receive and deal with telephone enquiries, referring telephone queries to a more senior colleague where appropriate. Taking accurate messages and relaying them to the appropriate person in a timely manner.
7. Logging calls with the Trust’s Service Desk for IT issues, building repairs, works etc.
8. Prioritise own workload and meet deadlines in order to meet the requirements of the post.
9. Monitor central supplies/provisions and order after discussion with line manager.
10. To have proficient keyboard skills and use Microsoft Office programmes such as Word, Excel and Outlook competently.
11. Accurately maintain and keep secure patient health and staff records/systems in line with policy and standard operating procedure, to ensure data quality, confidentiality and compliance with the current Data Protection Law
12. Maintain strict confidentiality regarding information concerning patients, staff and sensitive Trust Business at all times.
13. Communicate effectively and professionally on all levels.
14. Provide flexible administrative cover at the North Home First Team bases as required to meet the needs of the service.
15. Be responsible for any development necessary to fulfil the requirements of the post and own personal development plan.
16. Demonstrate relevant activities to new starters.
17. To ensure the work area is kept clean and tidy and that all display areas are kept stocked with relevant and up to date information/leaflets.
