The Community Transformation Programme is looking for a part-time (3 days per week) strategic operational / clinical lead to support delivery of the programme for 12 months (1 year fixed term contract). This role will provide leadership across the community programme, specifically supporting the business case development and associated implementation regarding the move away from CPA and the roll out of PROMS and implementation of the national access and waiting time standard. We are looking for an enthusiastic individual with a professional registration who has experience of working with clinical teams to implement change and new clinical practices.

The NHS Long Term Plan and NHS Mental Health Implementation Plan 2019/20 - 2023/24 sets out that the NHS will develop new and integrated models of primary and community mental health care. This new community-based offer will include access to mental health support within primary care networks delivered through a multi-agency team.
The successful post-holder will provide operational and clinical leadership to this wide-scale service development programme, working as a member of the transformation team and working closely with clinical networks, commissioners, as well as external system stakeholders.
The post-holder will need an in-depth knowledge and understanding of the national, regional and organisational context with regards to mental health community transformation, the requirement to implement patient reportable outcome measures (PROMs) and the requirements of the national access and waiting time standards for community services. They will also require an understanding of the national move away from the Care Planning Approach framework towards a personalized and collaborative care planning approach.

**Main Areas of Responsibility**:
- To provide operational and clinical expertise to the programme, to ensure pathway development and service redesign responds to patient need and is effective and safe;
- This will involve the provision of clinical expertise, service improvement knowledge and practice, facilitating the spread of evidence-based practice, and leading specific work streams to support delivery of the overall programme. This will ensure high quality standards of care are embedded in sustainable new systems and models of care both within the organisation, and across the wider health and social care economy;
- To lead on agreed work streams, supporting the design, delivery and implementation of plans, resource management and evaluation alongside operational and clinical teams;
- To identify new opportunities for innovation and transformation of clinical practice, ensuring robust evaluation of innovation and transformation is in place, ensuring benefits realisation.
- To support the programme SRO and Programme Director to deliver service transformation & sustainability by using an engaging, coaching and facilitative style and bringing specific skills in service improvement, evidence based practice and change management.
- To lead on ensuring programme plans/cases are formulated, understood and owned by all stakeholders and delivered utilising all available resources in an efficient and effective manner;
- To take lead responsibility for ensuring appropriate systems, processes and governance are in place across the system to enable the implementation of the transformed community pathway, with specific focus on PROMS, the implementation of the community access and waiting time standard and the move away from CPA;
- To communicate with and engage clinicians, managers and staff in developing a positive culture in respect of service improvement facilitation, leadership and ownership.
- Proactively manage the key risks and issues ensuring appropriate actions are taken to mitigate or respond.
- Avoid the destablisation of business as usual.
