**Main Areas of Responsibility**: - To provide operational and clinical expertise to the programme, to ensure pathway development and service redesign responds to patient need and is effective and safe; This will involve the provision of clinical expertise, service improvement knowledge and practice, facilitating the spread of evidence-based practice, and leading specific work streams to support delivery of the overall programme. This will ensure high quality standards of care are embedded in sustainable new systems and models of care both within the organisation, and across the wider health and social care economy; To lead on agreed work streams, supporting the design, delivery and implementation of plans, resource management and evaluation alongside operational and clinical teams; To identify new opportunities for innovation and transformation of clinical practice, ensuring robust evaluation of innovation and transformation is in place, ensuring benefits realisation. To support the programme SRO and Programme Director to deliver service transformation & sustainability by using an engaging, coaching and facilitative style and bringing specific skills in service improvement, evidence based practice and change management. To lead on ensuring programme plans/cases are formulated, understood and owned by all stakeholders and delivered utilising all available resources in an efficient and effective manner; To take lead responsibility for ensuring appropriate systems, processes and governance are in place across the system to enable the implementation of the transformed community pathway, with specific focus on PROMS, the implementation of the community access and waiting time standard and the move away from CPA; To communicate with and engage clinicians, managers and staff in developing a positive culture in respect of service improvement facilitation, leadership and ownership.

Proactively manage the key risks and issues ensuring appropriate actions are taken to mitigate or respond. Avoid the destablisation of business as usual. Key Working Relationships: - Operate effectively in a flexible and demanding environment and proactively engage with stakeholders; Communicate, proactively build good working relationships and provide information and advice to a wide range of internal and external stakeholders on a range of complex issues. Lead as an expert; integrating systems and managing effective working relationships with the appropriate stakeholders.

Drive and challenge each key working relationship to innovate and drive reform to achieve agreed objectives. Employ effective communication, negotiation and influencing skills to enable stakeholder relationships to deliver objectives over the duration of the programme with: - External organisations (such as GMMH and adult social care) to ensure seamless transition to the new pathways and systems;
- day work load in workshops, meetings, one-to-one communications and other events, comprising various parts of the business. Represent the organization and the system in sensitive and political situations, delivering difficult messages where required to high-level audiences.

Responsible for making recommendations, providing advice and preparing strategic reports/briefings as required. Nurture key relationships with senior and high profile individuals and responsible for the maintenance of networks. Ensure appropriate stakeholder engagement is in place and what information flows will be established and maintained. Policy, Service Development and Implementation: - Lead the implementation of specific work streams (moving away from CPA, the implementation of PROMS and the implementation of the national access & waiting time standard) to achieve the desired benefits, managing and monitoring the performance management process as necessary.

Develop and chair appropriate governance structures steering group / sub-groups to support delivery of outcomes; Work collaboratively with clinical teams to design and develop the implementation plan defining and implementing business processes that support the functions of the new system. Propose changes to and make recommendations for the programme as appropriate. Maintain a good knowledge of emerging policies from government departments. Manage system risks ensuring they are appropriately identified and controls and/or mitigation is in place, escalating to the programme leadership team Develop a recommended approach to implement the relevant policies (based on best practice research), inc.
