The relocation package is for anyone joining the Trust in a Clinical role who would need to relocate in order to take up employment with Cheshire and Wirral Partnership.

The role will involve completing a range of assessments that are relevant for the referrals received for people who have a Learning Disability. This will include completing communication assessments that can support the completion of Positive Behaviour Support Plans (PBSP). The post holder will also complete dysphagia assessments and other relevant assessments to support discussions and meetings to ensure that as a MDT we can be remain responsive to the diverse needs of the learning disability population.

All aspects of the role require the completion of the relevant documentation on SystmOne, the electronic system that is used by CWP so we can continue to evidence outcomes for patients, this includes the completion of care plans and risk assessments.

The team has two bases, Macclesfield and Crewe, and there is an expectation of seeing patients across both sites.

CWP provides health and care services for local people, including mental health, learning disability, community physical health and all-age disability care - including the provision of three GP surgeries.

All new clinical support staff will be required to have completed or working towards the Care Certificate (CWP In-house 2 day training to be completed within 6 months of start date).

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. You are encouraged to enrol for the DBS Update Service. A small annual fee of £13 per year applies.

New entrants to the NHS will commence on the minimum of the scale stated above.
