Job Introduction
Main Responsibilities
This is a demanding role requiring high levels of administrative and communication skills and the ability to work proactively as part of a team.
**Responsibilities will include**:
- Providing initial screening of all referrals into the Single Point of Access (SPA) from GPs, Service Users, Parents, Carers and other partner organisations
- Direct and prioritise incoming referrals to the relevant professionals and remedy any incorrect referrals at source
- Work with clinical colleagues to ensure patients are efficiency directed to the correct services in a in line with agreed ways of working
- Action call backs to referrers to ensure appropriate information is obtained, ensuring a high-quality referral
- Accurately and confidentially record all referrals for service users and carers on various client information systems.
- Signpost to the public with information on HCRG Care Group and alternative services provided by other statutory and voluntary agencies
- Collate, book, and send information and appointment letters to patients
We are looking for someone with knowledge of the principles of patient centred service delivery along with a working knowledge of data protection and patient and staff confidentiality requirements.
We are on the lookout for someone who is enthusiastic and motivated and can always showcase a caring and respectful attitude towards patients and staff.
