We provide community care in the Bristol and South Gloucestershire areas. As part of our expansion into Supported Living and Outreach services, we are recruiting Support Workers to work across Bristol and South Gloucestershire. We need compassionate, committed and kind people to provide person-centred support to adults with a wide range of support needs, including Autism, learning disabilities and mental health issues.

**Job Purpose**

**The Service**:
The service is for adults with a learning disability, Autism, mental health issues, those with sensory impairments and behaviours which challenge.

**The Role**:

- We are looking for an enthusiastic, passionate and dedicated Support Worker to join our team. This important role will support adults to thrive and have the best possible quality of life.
- As well as day time working hours, this role involves evening, weekend and overnight work.
- There is a requirement to support clients with their personal care needs, such as washing and dressing.
- You will support clients to access the community, provide mentorship, social opportunities, maintain relationships with family and friends, develop new skills, new hobbies and work towards independence.
- Our goal is for clients to be able to grow in confidence for leading an independent and fulfilling life.

**Duties and responsibilities**

To support the person(s) that you are paid to support:

- to live the life they want to lead, make decisions, have as many choices about their lives as possible and be supported to take appropriate risks.
- to make each day the best it can possibly be.
- to maintain existing skills and support to develop new ones.
- to support with accessing community facilities and take part in activities outside of their home.
- to support with maintaining the home, as well as cooking, shopping and budgeting.
- to support with maintaining overall good health, including attending health care appointments and taking medication.
- to support with personal care (e.g. washing, bathing, shaving, going to the toilet) when required.
- to support clients to achieve their goals and outcomes as set out in their support plans.

You will be expected to:

- treat clients with dignity and respect at all times.
- be aware of, and follow the procedure for, reporting any safeguarding concerns or accidents/incidents.
- keep accurate records.
- attend all required training.
- attend team meetings and supervision.
- ensure the health, safety and welfare of the people you support and your colleagues.
- work as part of a team.
- work with others involved in the client's life, including families and social workers.
- drive clients' vehicles where applicable and if you have a full UK driving licence.
- undertake any other appropriate duties as required by the person(s) that you are paid to support or management and that are appropriate to your role.
- perform your role in accordance with the organisation’s policies and procedures.
