Children's Continuing Care Case Manager

The contract will last around 12-Weeks with the likelihood of extension. This role is available on a apart-time basis with 30 hours per week available. Hybrid Working is available.

**Pay**:£25.00 LTD per hour + £250 Sign-Up Bonus
**Role: Children's Continuing Care Case Manager**
The post holder will be expected to be an effective member of the Continuing Healthcare Team, working flexibly to meet the changing needs of the health community.
- Supporting the Clinical Lead Nurse and Head of Continuing Healthcare in the development of a range of services for children with complex health needs.
- Undertaking specialist assessments for children with complex needs in relation to care packages, respite care and residential care placements.
- Collating and undertaking comprehensive and coordinated assessments for decision making for NHS Children's Continuing Care as part of a multidisciplinary team with families, and health and social care professionals.
- Promoting Personal Health Budgets and assist families to utilise their Personal Health Budget in accordance with national guidelines.
- Providing professional Nursing advice to Acute Hospital Trusts, Residential Homes and Social Services with regards to Continuing Care and Personal Health Budgets.
- Attending a multidisciplinary panel where arranged to present cases and provide clinical information.
- Case managing continuing care packages, ensuring timely reviews are conducted and ensuring care packages continue to be effectively commissioned and are cost effective.
- Assisting in maintaining a database for all patients assessed for continuing care.
- Developing strong working relationships with partner agencies including Acute Hospital Trusts, Local Authorities, the Independent Sector and others.
**Essential**
- Must be a Registered Nurse registered with the NMC (RMN/RNLD/RGN)
- Must have previous experience working with Children
- Must have a valid Driving License and Access to Own Method of Transport
