We are excited to reveal that we are looking for Care Navigators (band 7) to join our "Keyworking Programme" covering West Berkshire, from our base in Erlegh House, Reading.

The aim of the "Keyworking Programme" is to trial a new way of working to support children and young people with a learning disability and/or autism, who are at risk of admission to a mental health hospital.

As well as supporting children and young people you will also work with their families to make sure they are fully involved in their plans and they have the support they need at the right time, in a co-ordinated way.These are not clinical roles as such but strive to actively assess, navigate and co-ordinate the teams around the service user to bring about therapeutic intervention that is strengths-focused, evidence-based and tailored to meet the needs of the child/young person and their family/carer.

The working hours for these roles are Monday to Friday 9am to 5pm and we are open to both full and part time applicants.
