**About the role **The post holder will be responsible for supporting the daily running of the community virtual ward ensuring patients are enrolled within a timely manner.
**Responsibilities / main duties**:

- Manage onboarding of patients onto the Virtual Ward from several pathways.
- Ensure that referrals are managed in a timely way.
- Case entry of calls referred to the service.
- Liaise with all clinical staff to ensure their adherence to DHU procedures and standards.
- Update and maintain the Virtual ward dashboard.
- Provide administrative support to the Virtual Ward leads.
- Assist in general administrative and clerical duties and maintain manual/computerised records as required.
- To comply with all Company policies, procedures, and practices and to be responsible for keeping up to date with any changes to these.
- Provide assistance to new members of staff to ensure the maintenance of a high-quality service.
- Attend training as and when required to do so by management.
- Maintain strict confidentiality in relation to all issues concerned within the service and adhere to the requirements of the Data Protection Act 1984.
- Adhere to Health and Safety Policy.
- Notify Line Manager/s or Duty Supervisor of any circumstances which may affect the provision of a high-quality service.
- Report any complaints or incidents on the appropriate forms and provide further information if required in a timely manner.
- Keeping our patients, the public and ourselves safe and well is part of every role in DHU. To maintain high levels of safety all staff are encouraged to openly report incidents and any risk identified in Datix or where not possible to use Datix to report through the shift report.

Communications and Working Relationships
- Staff within DHU Healthcare and related organisation's.
- Clinical and managerial staff within participating Primary Care Networks.
- Clinical and other appropriate staff in local primary and secondary care services.
- Staff within local authority Social Services.
- Out of Hours and other related services within the local health economy.
- Ambulance services.

**Essential qualifications / skills**
- Good standard of general education including 4 GCSEs Grades A-C or equivalent including English and Math's.
- NVQ Level 2 or equivalent.
- Previous clerical experience
- Experience training others
- Working in an NHS/clinical environment e.g. hospital, GP surgery
- Ability to liaise with members of a multidisciplinary team.
- Excellent planning & organisational skills.
- Ability to prioritise workload to respond to changing demand.
- Excellent interpersonal & communication skills.
- Ability to deal with challenging behavior.
- Ability to provide excellent customer care.
- Knowledge of IT databases and computer systems.
- Analytical skills, ability to problem solve and make decisions.
- Accurate data entry.
- Excellent telephone manner.
- Able to work independently, with minimum supervision.
- This role supports both in and out of hours services, so evening and weekend cover on a rota basis may be required.
- You may also be required to travel to other DHU sites or bases.
- Understand teamwork and work within a team.
- Able to plan and organise workload.
- Can remain calm and professional in a busy environment.
