It facilitates discussions across the community and enables our patients to access care and support. It should be seen as an enabler across the health, social and community sectors putting our patients first. In the post simplistic form it gives patients and communities support in accessing effective and collaborative healthcare looking at them as a whole person. Wellbeing Hubs are facilitated by Wellbeing Advisors.

**Sheffield Walk in Centre **sees 77,500 patients annually through walk-in and those booked directly into the service through external system partners such as NHS 111. We have a close relationship with the local hospitals and work across the city with our external partners, to ensure we are providing high-quality care for our patients.
