The Diabetes team at Prince Philip hospital have a vacancy for a Band 3 HCSW. This HCSW role involves working in conjunction with Nursing, medical and other members of the multidisciplinary team within the Diabetes Centre.
The post holder will be responsible for the delivery of a high standard of cost effective nursing care and administrative duties, as part of the Diabetic Centre team. Although the post holder will at all times act as directed by the nurse in charge of the department, they will be competent to work without direct supervision within a framework that has been agreed by the Senior Nurse Manager

**You will be expected**:
- To undertake identified allocated tasks in relation to the care environment, the direct care of the patient / client as identified by a registered nurse and in accordance with the Health Boards policies and procedures.

Assist in ward work as directed from the Clinical Nurse Specialists.
Contribute to the development of the patients plan of care by assisting in tests, monitoring and procedures including:
Take and record patient observations (Blood Glucose Monitoring, BP, pulse, Temp, SaO2, peak flow, capillary blood glucose, urinalysis)

Prepare for, undertake and record care activities within the framework of a plan of care according to level of competence and as agreed by the registered nurse, e.g.:
- Monitoring of patients vital signs in accordance with Health Board Standards ensuring escalation with NEWS triggers
- Accurately record and reports back to the registered nurse on the assessments undertaken and risks identified.
- Reports any perceived changes noted in the patient’s condition to the Nurse-in Charge
- Works within Health Board policies and procedures as laid down within the Health Board and to observe the safety of the patient/clients.
- Undertakes other tasks appropriate to the level of competency, at the discretion of the Senior Nurse on duty.

Numerous locations providing mental health and learning disabilities services;
Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
