Our new Community Support Workers will join Home First North Team in delivering reablement as part of a collaboration between community, acute and social care providers.

You will need to demonstrate good communication skills and a positive approach to older people who may have communication difficulties or who may have challenging and complex health needs including frailty.

You will be part of a dedicated team and will be provided with bespoke training to support your development in this role.

The post holder will be expected to work with patients in their usual place of care this may also include Residential homes and nursing homes.

You will work as a part of a team to provide an 8am-8pm, 7 -day service for the reablement of patients with an expectation to cover 24 hour, 7 days a week service in the future.

As a Community Support Worker you can shape the lives of others.

**Main duties of the job**:

- The main purpose for this post is to provide a versatile and responsive role to support the Ageing well Urgent Community Response and Reablement standards in community services
- The post holder will be a valuable member of the community team to support the Ageing well clinical pathway, working within each locality, they will form a part of an Integrated Care Pathway for older patient
- Working as part of an interdisciplinary team, to facilitate holistic reablement for patients.
- Work under the guidance and clinical supervision of the locality team including ward and community staff.
- Must be able to provide, receive and feedback routine information which requires tact and persuasive skills.
- Ability to persuade patients to co-operate in their reablement programme; tact and re
