The Care Co-ordination Centre (CCC) is at the centre of the West Essex Health and Care Partnership transformation programme to support the delivery of the Out of Hospital Strategy.
- Its ability to be effective and deliver the best outcomes for West Essex citizens relies on the CCC being informed and proactive in the following:

- Holistic Overview of the adults previous care history to ensure the right care is facilitated
- Understand the needs of the adult and the services in West Essex to support earliest possible discharge from hospital, avoid hospital admittance, reduce escalating need and support the rehabilitation of the adult
- Multidisciplinary Team approach for complex adults led by the CCC
- Integrated Social Care, Community Health and Mental Health within the CCC
- Tracking of the adult through their transfer of care journey
- Understanding of system capacity to make the best decision for the adult
- Responsible for the sourcing of care
- Incorporates the TOCH responsibilities
- Manages system demand and informs future commissioning decisions
- This is a 2 year journey but with a good start
