This is an exciting opportunity to lead on the development and delivery of a Network Outreach Model of Care within Spinal Injuries, as part of a national transformation programme, funded by NHSE&I.

You will have a lead role in supporting the development of a seamless, coordinated, and timely approach to provide early response and enable specialist input to support patients with a range of spinal conditions including recent trauma.

Working across acute NHS organisations, you will support other health care professionals to provide early rehabilitation interventions and the provision of expert, evidence-based practice to spinal cord injured patients, their families and allied health professionals.

The applicate will ensure effective governance systems are in place to promote safe and productive clinical practice within STH and throughout the Network, identifying and escalating risks as appropriate.

We are looking for an ambitious, experienced, highly motivated innovative healthcare professional to join our team. Applicants must demonstrate a passion for service development, promoting best possible evidence based clinical practice for spinal cord injury patients and have the ability to engage across professional and organisational boundaries. Applicants must also have exceptional communication skills and the ability to lead, influence, negotiate to inspire a culture of positive change.

You will utilise advanced knowledge and clinical skills in spinal cord injuries to support the development of a Network Outreach model of care implementing a single network team across multiple acute hospital locations, spread across a wide geographical area ensuring safe, effective, and high-quality care is provided to patients and relatives.

Working closely with the Unit Matron, and clinical teams throughout STH and partner organisations across the Network to support early rehab and to identify alternative treatment options closer to patient’s homes.

Provide leadership, management, and a high level of clinical expertise within own area of specialism to prevent and offer an alternative to secondary admissions and facilitate timely transfer between different care settings.

Working with a high degree of autonomy and have professional accountability for the Network Outreach model of care that encompasses Outreach and Community Liaison, Discharge and Regional multi-disciplinary teams.

Ensuring the Network Outreach team, both internal and external to the Trust, possess the appropriate level of skill and expertise through development of an educational framework.

Ensure compliance with risk assessments, health and safety legislation including information governance, and quality assurance measures within the clinical areas and assist in the management of Health and Safety and Clinical Risk within the speciality, ensuring that all risk assessments are regularly reviewed.
