Job summary

We are looking for a talented, exceptional individual to join us as theClinical Lead for theSouth Yorkshire Integrated Stroke Delivery Network (SY ISDN). This new and exciting role will lead our Rehabilitation and Community workstream.

The post holder will support the development of rehabilitation services across South Yorkshire working towards regional alignment with the National ICSS model, alongside equitable and accessible Life after Stroke provision and robust secondary prevention interventions.

Pivotal to this role will be working effectively with key stakeholders across the region, including those from provider, commissioning, regional bodies and third sector organisations. The role will have a strong focus on workforce innovation, education and securing a future proofed, competency-based stroke workforce.

This role will be predominantly home based, but there will be access to offices and some travel across SY will be required.

This role is offered on a 12 months secondment or fixed term basis

Main duties of the job

This role will involve:

Using previous gap analysis data to further support the development of rehabilitation services across South Yorkshire, working towards regional alignment with the National ICSS model.

Developing an equitable and accessible Life after Stroke model across South Yorkshire

Securing strong engagement and partnership working across the region in order to develop and deliver Rehabilitation and Life after Stroke Services.

Close working with the SY ISDN Manager, Acute Clinical Lead and wider team to identify and implement improvements

Planning and delivering key workforce development initiatives focusing on collaborative workforce planning, innovative workforce models, different ways of working and new innovative roles.

Identifying and developing provision of secondary prevention opportunities to reduce recurrent vascular events.

This vacancy is advertised on a fixed term contract basis for a period of 12 Months due to the post being subject to national funding, reviewed annually. The fixed term contract status of the post holder will be reviewed in accordance with the Trust's legal obligations regarding fixed term contracts.

The ISDN achieves this this by bringing key stakeholders together, tofacilitate a collaborative approach to service improvement of the whole stroke pathway ensuring a patient centred, evidence-based approach to delivering transformational change. SY ISDN is hosted by Sheffield Teaching Hospitals NHS Foundation Trust.

Educated to degree level in a relevant clinical area nursing, physiotherapy, occupational therapy, SALT with a Master's or equivalent qualification or experience Member of relevant professional body Evidence of continuing professional development.

Significant experience of working within stroke rehabilitation as a clinician. Experience of working in complex systems and working with collaborative decision making groups or environment.
