Crisis Resolution Team Band 5 Clinical Practitioner

An exciting opportunity has arisen for band 5 registered practitioners to join the Camden + Islington Crisis Resolution Teams

This post is open to RMNs, social workers, and occupational therapists (including Newly Qualified clinicians) for which we are seeking proactive, dynamic and skilled staff to work in our fast-paced multidisciplinary team. We are seeking applicants who possess the skills and enthusiasm to take on the challenges of a changing, modern day mental health service.

We are a 24 hours service, 365 days per year intensive crisis assessment and home treatment service providing mental health crisis care to our local adult residents in the vibrant and diverse boroughs of Camden and Islington. Our Crisis Teams all located close to good transport links. As we provide mental health services to a diverse and multi-cultural population we endeavour to reflect this in our workforce. We are fortunate to work closely with several crisis houses, including a third sector crisis house. We work with a wide range of professionals in an multidisciplinary team including peer support workers. We are enthusiastic about Quality Improvement projects which we regularly take part in to improve patient and staff experience.

Working as a Band 5 clinical practitioner in the CRT will help you develop leadership skills, autonomous decision making, and dynamic risk assessment skills, as well as opportunities to progress in your career. You will have the opportunity to work across 4 different aspects of the crisis pathway that the service delivers which include:
Crisis -Single Point of Access

A 24/7 busy telephone service offering support for people in crisis as well as triaging referrals from professionals and the general public to the crisis teams.

Assessments

Undertaking holistic assessments of people in crisis and deciding on collaborative treatment plans as an alternative to acute inpatient care or signposting to appropriate other community or third sector services

Home Treatment Team

Working as part of a close-knit team to deliver high-quality person-centred care in the least restrictive setting. You will be using and offered training around the use of CBT/DBT, de-escalation techniques, managing risks, brief supportive counselling skills to work towards shared treatment goals with the service user within their social system.

Night Crisis Service

There is crisis service provision at night which combines aspects of all 3 services to assess and support people in crisis during night time hours. Currently, the expectation is for the post holder to work approximately 2-night shifts per month.

Reporting to the Team Manager or designated supervisor the postholder will undertake a range of necessary tasks, including taking and screening referrals, prioritising referrals, identifying appropriate action in processing referrals, carrying out assessments, developing and agreeing care plans with service users, implementing and evaluating care plans, and identifying and referring on to appropriate support services for service users’ ongoing care. This will involve delivering care and interventions appropriate to the post holder’s level of experience and competences without direct supervision.
- To contribute to the multi-disciplinary team comprehensive, culturally

appropriate assessments to establish eligibility for health and social care
services.
- To undertake continuous risk assessment and to contribute to effective risk management. At all times to act in accordance with trust policies in relation to risk assessment.
- Recognising and valuing service users as individuals the post holder will ensure that the care provided respects equality and diversity that is supportive of service users and their carers. This will include encouraging service users to accept an optimum level of responsibility for their individual programme of care and with their consent, where appropriate, seek the cooperation of friends relatives and/or carers.
- Utilising agreed frameworks and guidance such as NICE or NSF; evidence based best practice and or local policy and procedure, as well as a range of profession specific tools, the post holder will actively assess, plan and

determine the care needs of service users. They will undertake and contribute to specific or multi-disciplinary team reviews of such care.
- To undertake the physical health care assessment and or on-going physical health monitoring / review of designated service users, recording any changes in the service users’ physical health, reporting these to appropriate members of the team and acting appropriately on the advice given.
- To identify and provide information to service users that promotes their health and wellbeing. Directing them as appropriate to others / other services in order to address any identified needs.
