Pcn Care Coordinator

**About the Role**:
GP Healthcare Alliance is a local federation of 18 GP practices in South East Essex. In partnership with Southend West Central Primary Care Network (PCN), we are hosting an innovative service to recruit a care coordinator to support the PCN additional roles team, to provide a central co-ordination role for patient care planning, across member practices.

This role will be working autonomously but will have the support of a multi-professional primary care team, made up of doctors, nurses, and clinical pharmacists. We are looking for a proactive individual who is passionate about improving patient care.

The post-holder will work within a multidisciplinary team, providing a central co-ordination role for patient care planning within our care homes and PCN aligned community teams (PACT).They will support the delivery of the PCN engagement workstream with the local population by initiating contacts and developing, and managing, relationships across the network.

The role is full-time, 37.5 hours per week, and covers the Southend West Central Primary Care Network, which is made up of the following practices:

- Prince Avenue Surgery
- The Valkyrie Surgery
- Southend Medical Centre
- Dr Bekas Medical Centre
- Scott Park Surgery

The appropriate individual will have an NVQ Level 2 in business administration, or have relevant transferable skills.

Significant SystmOne experience is important, in order to manage rotas and update patient records accordingly.

GP Healthcare Alliance is a company, originally formed as a GP federation, made up of 18 practices who work together to provide healthcare for our practices and patients, in Castle Point, Rochford and Rayleigh, Essex. All practice members remain independent contractors, but work collaboratively to improve healthcare services within our area.
