**What you’ll do**:
**About you**:

- Build supportive, trusting relationships with customers
- Manage a caseload of customers, supporting key customers to set personalised goals in the form of a Support Plan that incrementally works towards greater levels of independence and positive move on from the service
- Conduct regular key work sessions that are innovative and engaging in order to achieve Support Plan goals
- Ensuring ongoing assessment and management of risks associated with customers ensuring safe and effective ‘positive risk taking’
- Working proactively with other members of the team to handle the service caseload and support an effective team approach to meeting each customer’s identified needs
- Contribute to a positive service environment, ensuring the service is supportive and a place of safety and stability for customers. This will include working flexibly and being proactive in delivering, reviewing and arranging effective support for emerging needs and risks in order to prevent escalating concerns
- Deliver all aspects of support to enable a customer to develop independent living skills as appropriate to the individual needs of the customer. This may include practical tasks that are essential for independent living such as handling mail and supporting phone calls. This may also include delivering personal and physical care, including but not limited to support with personal hygiene, meal preparation, medication management and personal finances as required
- Support customers to undertake domestic tasks, including practical assistance where they have not yet developed the skills, to ensure a high quality living environment that promotes customers’ dignity, respect and allows customers to feel safe and welcome in their own home. In some cases, this will include hands on support to maintain customers’ living standards, including enabling a clean, safe and homely environment
- Provide support and guidance to other front line staff in your particular area of specialist expertise. This area of expertise will include working with those that have histories of substance misuse, complex trauma, personality disorder and/or offending behaviour in to mental health related needs
- Recognising associated indictors of relapse/concern
- How to employ different approaches to meet particular needs
- Capacity building and development of knowledge
- Engendering an understanding of particular care-planning processes and models of care including harm minimization strategies
- Understanding the impact of complex needs that present additionally to mental health related needs
- Contribute to the relevant internal specialist forum
- To work to the Drug and Alcohol Nation Occupational Standards (DANOS) of support where relevant. This may include undertaking drug and alcohol screening tests where required
- Proactive development of links with local statutory and voluntary organizations to provide a range of engagement opportunities for customers in the community
- Involving customers in the design, development and delivery of the service
- Empowering customers to maximise the income through employment opportunity and/or ensure they receive the service and benefits they are entitled to
- Encouraging and enabling customers to meet reasonable expectations of being a tenant, including to pay their rent and ensure that rent accounts are managed
