From substance misuse counsellors to social workers, specialist Doctors, psychiatrists, nurses, outreach workers, and administrative staff, each member of the team plays a unique and vital role in achieving their mission. We have an exciting opportunity for a **Rough Sleepers** **Recovery **Coordinator** **who has the skills and experience to assess and engage with our service users, ensuring that they access the right intervention at the right time, in a way that best meets their needs.

**Salary**: £27,449.52 to £31,529.41 per year
**Hours**:37.5 per week
**Contract Type**: Fixed term - 12 months
**Responsibilities**:
**About the Role**:
- In your role as a Homeless Recovery Coordinator, you will:
- Establish effective communication and referral pathways for individuals who are homeless and in contact with other health and care agencies.
- Attend relevant multi-disciplinary team meetings to support links to health and social care services, peer support networks, community assets, and mutual aid.
- Collaborate closely with the local authority, the health and social care sector, partners, and any provisions linked to homelessness to improve joint working.
- Liaise with other services involved in the care and support of individuals to ensure the appropriate and secure transfer of all care needs.
- Coordinate the transfer of any accommodated out-of-area individuals, including referral to and liaison with their new drug and alcohol service.
- Enhance partnership working with primary care, mental health services, housing, and other voluntary organisations supporting rough sleepers.
- Ensure there are culturally appropriate interventions for hard-to-reach individuals from the Eastern European and Sub-Saharan African rough sleeping communities.
- Utilise networks and pathways to broker support from strategic partnerships and other providers.
- Ensure continuity of care for those moving out of the area.
- Hold cases for a minimum of 4-6 weeks following a transition to ensure the service user moves between the two services successfully.
**About You**:
To excel in this role, you should:
- Focus on engaging with people who have a history of rough sleeping and substance misuse.
- Have some experience in engaging with individuals with multiple and/or complex needs and demonstrate a determined approach to finding solutions.
- Possess some knowledge of areas such as homelessness, mental health, the criminal justice system, and substance misuse.
- Treat every person who comes into our service as an individual, so being flexible, curious, and a good listener is crucial.
- Enjoy record-keeping and attention to detail.
- Embrace the dynamic nature of the role, where no two days are the same.
