Collaboration and Communication: Work closely with GP's, nurses, social prescribers and other healthcare providers to ensure coordinated and comprehensive care for patients and to communicate the patient's status with the relevant team in the surgery. Patient Advocacy: To advocate for the patients needs and to ensure they have access to appropriate services and resources.

**Palliative and End-of-Life Care**: Provide palliative and end-of-life care, offering comfort and support to patients and their families during challenging times. Overall, the role of a home visiting paramedic in primary care contributes to improving access to healthcare, reducing hospital admissions, enhancing patient outcomes and promoting wellness within the community. It requires a combination of medical skills, empathy, effective communication, and a commitment to delivering high-quality patient centred care.
