The Health Inclusion Team provides a nursing and health visiting service to a transient population of people who may be living in homeless accommodation, women’s refuges or may be Asylum Seekers, Refugees, Gypsies or Travellers.

As a health visitor you will be expected to undertake family health assessments and child health surveillance in accordance with the Healthy Child Programme for adults and children, 0 to 18 years.

You will work in partnership with children, families and individuals to provide high quality effective healthcare, to prevent ill health, promote positive health and reduce health inequalities. You will contribute to the development of the service for Homeless people, Gypsies and Travellers and Asylum Seekers and Refugees.

The team works closely with a range of agencies to ensure a strong and coordinated service working within the NMC Code of Professional Standards and Clinical Governance Frameworks.

To identify, prioritise, develop and implement programmes of care to meet individual and local needs, taking into account cultural diversity and ensuring prioritisation of work load as necessary

To work with families in a variety of settings including home visits and clinic, providing evidence based advice and support.

To undertake family health assessments and child health surveillance in accordance with the Healthy Child Programme to include adults and children and young people, 0 to 18 years.

To provide relevant information to families, to assist informed choices about their own and their family’s health.

To operate within Sheffield Safeguarding Children Board and SHSCFT Child Protection and Safeguarding Procedures. To maintain and update knowledge around safeguarding children and young people and ensure that practice complies with local and national policies and legislation. To work closely with the Sheffield Safeguarding Advisory Service and other key agencies to ensure the highest level of protection for children at risk. To attend safeguarding or child in need meetings and provide reports as necessary.

The service aims to carry out assessments of health and social needs to identify any gaps; referring to other agencies as necessary. The team offers the Healthy Child Programme for children and pregnant women; health assessments for young people and adults as well as support in accessing mainstream Primary Care and other key services.

Leadership

To participate in clinical supervision. To participate with the manager in personal

appraisal to identify professional and personal development. To participate in relevant training and take personal responsibility to keep professionally up to date with practice.

To appropriately delegate care to members of the multi skilled team ensuring oversight of care delivered and supervision.

To take on the role of clinical supervisor, coach and mentor for peers, colleagues, students and other staff as appropriate.

Clinical

To encourage appropriate access to Primary Care Services for homeless, traveller and asylum households

To maintain clinical competency and a portfolio of evidence of continuing professional development. To comply with NMC guidelines and relevant legislation to ensure that clinical practice is of the highest standard.

To prescribe medicines as appropriate in accordance with the NMC prescribing standards within the Nurse Prescribing Formulary.

To manage time effectively, prioritise own workload and supervise the practice of other team members (including bank and agency staff), both directly and indirectly as appropriate.

Communications

To maintain accurate, legible and contemporaneous electronic records in accordance with Trust and NMC guidelines and policies.

To liaise effectively with other members of the team and other relevant agencies.

To make ongoing referrals for support as necessary working with other care providers.

To maintain effective working relationships with social care, safeguarding services and children’s centers.
