We have an exciting opportunity in our Cornwall services for an Hospital Outreach Worker to join our busy and dynamic team based in Truro.

This is an exciting and challenging role that works from a trauma informed perspective based in the community covering all of Cornwall. Due to the outreach basis of this role, a full driver's licence and access to a vehicle is required.

The Hospital Outreach Service provides a rapid response to individuals who are frequent attenders at the Royal Cornwall Hospital in Truro and have issues related to substance use.

The team works within a multi-disciplinary model of practice with partners in safeguarding, housing and homelessness services, domestic abuse services, as well as primary and secondary health care and mental health providers.

We offer a diverse package of training opportunities and there are plenty of chances for ongoing personal development.

This role is primarily based in the community, but on occasion you will support clients who are inpatients in Cornish Hospitals.

**About this job**

You will work as part of a team with the aims of:

- understanding your clients
- keeping them safe
- creating a plan they can achieve with your support
- checking in with them and reviewing their progress
- keeping accurate and timely records of your contact with them
- working effectively with other social care providers e.g. Housing, to cover all aspects of their successful recovery
- continuously learning and reflecting to improve the outcomes for your clients
- supporting your colleagues across the service to deliver excellent care

**REQUIRED SKILLS**:
To be successful in this role, you will:

- have a clean driving licence with access to a vehicle
- understanding of the barriers that prevent people with substance use from accessing health care
- experience and understanding of trauma informed practice
- ability to manage own caseload
- understanding and experience of the safeguarding process
- commitment to challenging issues of discrimination
- have some experience of working in a health, social care or related setting
- have the ability to build and maintain effective relationships with clients and colleagues alike
- be able to work effectively in a team, supporting others when needed
- be resilient under pressure and able to prioritise you work
- have good oral and written communication skills
- be comfortable using IT and digital media
- be organised and able to manage your time effectively
- able to be flexible with your working hours on occasion
- be happy to support other With You locations when needed
- meet our safeguarding requirements of a satisfactory enhanced DBS check.

**DESIRABLE SKILLS**:

- have previous experience of working with people who misuse substances
- knowledge of health and safety, including assessing client in high-risk situations
