Support the Team Leader in the daily operational management and leadership of the Community Children’s Diabetes Nursing Team ensuring the delivery of high quality health care, education and support to children and families within the home, school and in various community settings. Deputise for the Team Leader in his/her absence.
- To initiate and co-ordinate appropriate services in order to facilitate the early discharge, and manage children and young people newly diagnosed with Diabetes at home.
- To support the development of ambulatory care and promote the ethos of empowering children, young people and their families to develop the knowledge, skills and expertise to enable them to become self-caring, independent and manage their own condition at home wherever possible.
- To work as a member of the Children’s Diabetes multi-disciplinary Team; providing a holistic and developmental approach to diabetes from infancy to adolescence.
- To work closely with consultant team to optimise outcomes for children and young people with diabetes both as a primary condition and as a complication of conditions such as endocrine disorders.
- To provide in-reach support to ward areas to ensure that newly diagnosed diabetics receive appropriate treatment and advice

Support the team leader with the direct care and management of children and young people with diabetes.

Demonstrate high standards of self / time management and in the absence of the Team Leader support junior team members’ work/life balance / level of activity through the monitoring and collation of monthly records of hours worked. To practise autonomously with confidence and expertise

In the absence of the Team Leader, undertake daily evaluations of the team’s workload in order to organise, prioritise home visits to children, and ensure fair allocation and distribution of work among team members taking account of staff experience, skills, training and visit location.

To contribute to the development and maintenance of a cohesive team that ensures the wellbeing of all of its members through supportive relationships, working practices, specialist supervision, training and maintenance of appropriate professional boundaries.

To work as a member of the diabetes team providing assessment of care needs as well as development, implementation and evaluation of programmes of care/education for children/young adults and their families, with diabetes.

With support from the Consultant and other members of the MDT team to optimise treatment regimens, interpreting blood glucose and ketone results and adjusting insulin treatment within an agreed specific protocol.
