The post holder will be a valuable member of the multi disciplinary Intensive Dementia Service team (IDS). To work as a member of the Intensive Dementia Service, providing an alternative to hospital admission for people with dementia.

Working in the service offering specific input tailored to the needs of people who experience challenging behaviours due to memory/confusion, in order to improve their quality of life, maintain their existing skills and provide carer support.

When working with all cases, a warm, sensitive and stimulating environment needs to be created and you will need to demonstrate the ability to work as part of a team. Flexibility is a key quality, and you must be able to respond to clients in an appropriate manner. You will have good communication skills relating with clients and the carers and be able to demonstrate an ability to understand their needs and where possible, be able to help the carers.

As well as taking pride in caring for our patients, we also prioritise caring for our colleagues. You will be part of a compassionate team who understand the challenges that arise when working and the importance of supporting one another through these.

There is a requirement to work a shift pattern that provides 8:00 TO 20:00 7 days a week, 365 days of the year.

This position requires a full UK driving licence.

To work as a member of the Intensive Dementia Service, providing an alternative to hospital admission for people with dementia.

The post holder may be required to work flexibly to meet the needs of the service.

To provide a supportive role for carers and patients accessing the Intensive Dementia Support Service. To carry out assigned tasks as identified by the registered nursing staff in relation to patients’ needs.

You will be required, to visit clients’ in their homes/care homes so it is essential you have a driving licence and access to a vehicle.

You will also have some specific administration responsibilities therefore IT skills would be an advantage.

- To be able to follow and demonstrate the delivery and implementation of care plans, under the direction of registered staff
- To accept responsibility for an allocated group of patients each day, carrying out specific delegated tasks, whilst supporting and reporting to the registered staff member
- Being an active member of the multi-disciplinary team in assisting and engaging patients in organised, planned meaningful activities for patients; monitoring mental health and wellbeing
- Possess an awareness and understanding of what is normal concerning their patient's wellbeing and report that which is out of normal parameters to the registered professional
- Recognise risk in relation to care provision and will escalate risks accordingly
- Have an understanding of mental capacity and respond appropriately
- To have an understanding of the effect of physical health on mental health.
