**Description**:

- To work within the Mental Health Review Team within Adult Social Care.
- To review and/or reassess the social care needs of a number of residents in receipt of packages of care being transferred from the Mental Health Trust to Adult Social Care following the ending of the S.75 arrangements
- To work within the needs assessment, and contribute to the care plan and person centred plan to enable and empower individuals to make informed choices and achieve personal goals

**What is required?**
- Level 2 or above e.g. NVQ or Diploma in Health and Social Care or Promoting Independence
- Experience within a similar role
