Job Title: Senior Social Worker - CMHT (Community Mental Health Team)

Location: Caerphilly, Wales

Rate: £35 per hour

Job Purpose:
As a Senior Social Worker Practitioner in our Community Mental Health Team (CMHT), you will play a crucial role in providing support and care coordination for individuals experiencing severe and enduring mental health needs. Your responsibilities will include conducting assessments, coordinating care plans, participating in assessment clinics, and fulfilling duty desk duties. Additionally, if qualified and warranted as an Approved Mental Health Professional (AMHP), you may undertake Mental Health Act (MHA) assessments on behalf of the Local Authority.

Key Responsibilities:
Conduct comprehensive assessments of individuals mental health needs.

Develop and implement care plans in collaboration with service users, their families, and multidisciplinary teams.

Provide ongoing support and interventions to promote recovery and well-being.

Participate in assessment clinics and contribute to the duty desk rota.

Undertake Mental Health Act assessments as an Approved Mental Health Professional (if qualified and warranted).

Maintain accurate and up-to-date records in accordance with statutory requirements and organizational policies.

Work collaboratively with other professionals and agencies to ensure holistic support for service users.

Provide supervision and support to junior staff members and students as required.

Contribute to service development and improvement initiatives.

**Requirements**:
Qualified Social Worker with registration with Social Care Wales.

Substantial experience working within a mental health setting, preferably within a Community Mental Health Team.

Demonstrated understanding of the Mental Health Act and experience in conducting MHA assessments (preferred).

Ability to work autonomously and as part of a multidisciplinary team.

Excellent communication, assessment, and care coordination skills.

Commitment to ongoing professional development and evidence-based practice.

Full driving license and access to a vehicle for work purposes (preferred).
