Job Title: Social Worker - Adults

Client: Caerphilly County Borough Council

Location: The Rhymney Integrated Health and Social Care Centre, NP22 5PW

Role Information/Duties:
Caerphilly County Borough Council is seeking a qualified and dedicated Social Worker to join the North Older People Team at the Rhymney Integrated Health and Social Care Centre. This is a fixed-term, full-time position, offering an exciting opportunity to make a real difference in the lives of older adults within our community.

About the Role:
The North Older People Team is responsible for providing support and care to individuals aged 65 years and over, residing in the northern part of the county borough, who have physical disabilities and/or mental health needs. As a Social Worker in this team, you will be tasked with assessing and reviewing the needs of service users, as well as coordinating managed care and support services.

Key Responsibilities:
Manage a caseload of service users, ensuring regular reviews and effective risk management.

Contribute to the duty function on a weekly basis, providing office-based support.

Undertake integrated assessments, mental capacity assessments, and risk assessments.

Develop care and support plans to manage risk and achieve desired outcomes.

Protect older adults from neglect and abuse, in accordance with relevant legislation and policies.

Collaborate with a multidisciplinary team of professionals, including district nurses, GPs, and others.

**Requirements**:
Qualified Social Worker with relevant experience in adult social care.

Up-to-date knowledge of current legislation and policy in Wales.

Strong assessment skills and ability to manage varying priorities.

Excellent communication and interpersonal skills.

Commitment to achieving high practice standards and delivering effective social work services.

Ability to work collaboratively in a team-based environment.
