An exciting opportunity has arisen to join the ward clerk team on the Clinical Decisions Unit and Same Day Emergency Care unit (SDEC) at the Glenfield Hospital

The successful applicant should be computer literate and previous experience as a ward clerk within an NHS environment would be an advantage, however training can be given. A working knowledge of Trackit, Nerve Centre and Patient Centre would be beneficial.

The post requires you to have excellent communication skills and customer service abilities as you will be dealing with patients and their relatives.

You will be require to work using your own initiative, managing your own workload as well as being part of the Clinical Decisions Unit team.

You will be working a 7.5 hour day monday to friday on a rota basis between CDU and SDEC, ensuring that the needs of the unit are covered.

To support the ward staff and senior clerical staff in the administrative and clerical processes and procedures associated with the running of a clinical service, in support of excellent care for patients and their carers.

Capturing all necessary patient administration data for both emergency and elective patients by ensuring completion of the appropriate admission, transfer or discharge documentation and its input to PATIENT CENTRE/PAS via an appropriate computer terminal.

This will include assisting medical staff with the completion of data capture, booing patients in/out of wards, updating the bed states, checking the accuracy of the PAS information, tracking patients’ notes and maintaining patient notes and maintaining medical records.

Patient Information
- Using the Trust’s IT system, ensure that the patient’s case notes and documents are available on the ward for the patients
- To make up temporary file for patients admitted direct to the ward i.e. emergency admissions or out of hours for Admissions
- Complete front sheets of notes showing dates of admission and
- To maintain all filing up-to-date.
- Ensure that patient’s notes are complete for ward
- To book in and out medical notes on the Patient Centre system - training will be arranged.
- To trace patients on Patient Centre. If the patient cannot be found, register the patient using the completed admission
- To source and retrieve patient case notes from within the Trust and from other hospitals, liaising with the Medical Records Department as appropriate, checking and updating patients and GP details when received, to ensure accuracy and minimise errors.
- Ensure addressograph labels are available and attached to all medical and nursing notes, charges and prescription
- Enter accurate and timely patient information on Patient Centre ensuring accurate clinical
- Obtain patients notes required for pre
- assessment clinics and doctors

Receptionist Function
- Act as the ward receptionist to all visitors; give directions and advice as
- Answer telephones, staff bleeps and bleep medical staff as
- Issue car park letters to parents/relatives as

Patient Flow
- Check and update the ward bedstate using Patient
- Ensure nerve centre is up to date
- Ensure outpatient appointments are made at a variety of hospitals and clinic locations. and the card completed for those patients with follow-up clinic

Discharge
- Ensure that discharge summaries (written by Doctors) are completed
- Ensure that the laboratory results to be filed in the case notes after being checked and signed by
