If successful, you will be working within the Recovery Bristol Partnership delivering community health services in Bristol and Avon and Wiltshire Partnership Trust (AWP).

**Job Purpose**
- Work collaboratively with the service users and mental health professionals to navigate and coordinate services (both social and clinical interventions)
- connecting into community resources, building resilience and improving wellbeing.
- Work within a recovery and psychologically informed approach, taking a holistic view to support people’s recovery to live fulfilling lives. Ensure that services are tailored to individual’s cultural and social needs.

**Key Duties**
- Case manage and co-ordinate support referring and liaising with professionals, agencies and other parties
- Implement and review Recovery plans, Crisis and Plans, in partnership with service users, in line with best practice
- Identify, assess and manage risk to ensure risk is minimized.
- Have up to date safeguarding training, ensuring that Safeguarding policies and procedures are fully adhered to
- Enable delivery of the recovery plan and to enable service users to make informed decisions about their mental wellbeing
- Work in a culturally sensitive way for all service users, tailoring to meet their individual needs, challenging stigma and discrimination
- Manage caseload documentation effectively and ensuring the information is up to date.

**Person Specification**
- Experience of working with people with mental health or complex needs
- Experience of working with people who have multiple needs e.g homelessness and drugs and alcohol
- Carrying out risk assessments
- Knowledge of health and safety issues
- Knowledge of safeguarding in relation to adults children and young people
- The ability to travel to several appointments daily (including in rural areas with limited/ unreliable public transport) is essential
