An exciting opportunity has arisen to join an existing nurse led service within a multidisciplinary team based at Bournemouth Hospital but covering Dorset that cares for patients with Liver Disease and Hepatitis C.

Our patient group often have multiple complex care needs and face health inequalities and issues with accessing care due to drug and alcohol addiction issues, homelessness and mental health problems.

The role will involve being part of initiatives aiming to enhanced care of patients with Decompensated Liver Disease and the Dorset wide Hepatitis C Elimination Programme as well as projects looking at early detection of liver disease and Primary Liver Cancer.

We are looking to appoint an individual with either a background in care administration and coordination with a desire to be more patient facing or an applicant with established experience as a care assistant caring for this patient group in a healthcare setting.

**Locations**: Royal Bournemouth and Poole Hospital - this is a cross site position.

**Interview Date**: TBC

The post holder will work with the Liver Nursing and Homeless Care team of nurses to advocate for patients, coordinate care and effectively engage patients with complex needs undergoing care and in particular treatment for Chronic Hepatitis C.

With appropriate venepuncture training, the post holder will take blood samples, including from those patients with poor venous access and point of care testing including (dry) blood spot testing. Using clinical skills developed through specialist training carry out fibroscanning, assessments including blood pressure, height, weight, frailty assessment and to calculate FRAX and FIB4 scores.
