**Job Summary**:
**Responsibilities**:

- Develop individualized care plans for patients, taking into consideration their specific needs, preferences, and medical conditions.
- Conduct comprehensive assessments of patients' physical, emotional, and cognitive abilities to determine appropriate care interventions.
- Administer medication to patients according to prescribed dosages and schedules, ensuring compliance with all safety protocols and regulatory requirements.
- Monitor patients' health status closely, identifying any changes or concerns, and promptly report them to the appropriate medical personnel.
- Provide assistance with activities of daily living (ADLs), including bathing, grooming, dressing, toileting, and mobility support, as needed.
- Foster a supportive and nurturing environment for elderly residents, promoting social engagement, meaningful activities, and emotional well-being.
- Collaborate with healthcare professionals, family members, and other caregivers to coordinate and implement comprehensive care plans.
- Document all care-related activities accurately and maintain up-to-date records of patients' progress, treatments, and vital signs.
- Provide emotional support and companionship to patients, demonstrating empathy, patience, and understanding at all times.

**Qualifications**:

- Proven experience working in senior care facilities or similar settings, with a focus on dementia care.
- Strong understanding of geriatric care principles, including the challenges associated with aging and cognitive decline.
- Proficiency in creating personalized care plans and conducting assessments for elderly individuals.
- Excellent medication administration skills, with knowledge of common medications used in the treatment of dementia.
- Compassionate and empathetic demeanor, with the ability to build trust and rapport with elderly patients.
- Effective communication and interpersonal skills, with the ability to collaborate effectively with interdisciplinary teams.
- Ability to work independently and make sound judgments in a fast-paced environment.

**Salary**: £30,000.00-£37,000.00 per year
