**Role Purpose**:
To manage a team supporting families during hospital discharge, meeting defined discharge timescales. Sourcing care home placements/packages of care for patients with a variety of needs, including complex physical disabilities, challenging behaviour andmental health. To ensure a safe & quick discharge in line with the patients care plan, whilst supporting families often during a great time of stress.

This is a high-quality service that reduces unnecessary delays in the discharge process.

**Responsibilities**:

- Manage, support, coach, mentor and develop your team. Managing a small team of both office and field-based advisers.
- Leading and motivating teams to achieve their full potential and KPI’s.
- Ability to review data outcomes and react/report as defined by the results of the data outcomes.
- People management responsibilities, including recruiting of staff through interview process, probation reviews, disciplinaries, managing sickness and absence to include return to work interviews where relevant.
- Provide timely monthly or weekly KPI and contract performance reports to client
- Build relationships with people across many departments, internal and external and across all levels
- Updating and maintaining client records on our bespoke in-house database.
- Attend team meetings and training as required.
- Responsible for new opportunities to drive the business forward and discuss with line manager.
- Do presentations and meetings as required to new teams and professionals.
- Deputise for Business Manager in their absence, in accordance with the needs of the business.
- Manage ad-hoc project work to suit the needs of the business

- Communication: Articulate with written and verbal communication skills able to liaise with all directorates and departments, including patients and a range of professionals on sensitive matters.

- Motivated: desire and passion to succeed. Proactively manage a team, and deliver tangible results that add value

**Experience and/or Qualifications**:

- Knowledge and experience working within a fast paced environment with constant multitasking whilst delivering a high standard of customer care.
- Experience of achieving KPI’s and working to strict deadlines.
- Demonstrable administration skills.
- Knowledge of Microsoft Word and Excel
- Leadership skills, including handling conflict resolution.
- Coaching, supporting and rewarding a team.
- Ability to delegate effectively.
- Understanding of hospital and community structures Social Services would be advantageous
- Understanding of confidentiality, data protection, Data Protection Act 1998 and safeguarding and working to protocols and procedures including Health & Safety Policy and Fire Policy and Procedures.
- Full Driving Licence
