The Pathway Support Worker Coordinator (PSW) will act as the point of contact for the Multidisciplinary Team (MDT) staff, promoting an effective and smooth discharge for all patients on their designated areas, Wards, ED/SDEC/Pathway Hub.

They will be responsible for highlighting any referrals which the PSW will need to complete. They will then ensure that all aspects of the patient’s discharge is progressed and escalate when there are changes or delays to the Estimated Date of Discharge (EDD), patient needs or clinical condition.

Alongside these elements the post holder will be responsible for ensuring accurate data is captured on all data bases to fulfil all Trust reporting requirements. The Pathway Support Worker Coordinator will participate in a 7/7 day working pattern which includes weekend working and covering out of hours Rota which reflects the needs of the service.

The post holder will assist with all aspects of early discharge planning & admission avoidance and participate with MDT to ensure seamless transfer from acute hospital to appropriate destination. Work with the Multidisciplinary team to provide safe patient focused care.

This will require the deployment of communication, interpersonal and practical competency skills, including excellent IT skills.

A key part of the role will be to ensure there is early communication with families/carers/care homes etc, to ensure all relevant information is collated and potential barriers are highlighted so they can be addressed and solved prior to when the patient leaves hospital and mitigate failed discharge.

Our values give us all a common purpose and help patients, as well as their family and friends, to know what to expect of everyone they meet when they visit Dorset County Hospital Foundation Trust (DCHFT).
- The post holder will facilitate patient flow within the patient journey, working alongside the MDT, Partner Agencies and Voluntary services to identify and minimise any delays. These include chasing diagnostic tests, TTO medication, additional assessments, and social issues.
- The post holder will be required to liaise closely with clinical teams and work collaboratively with all agencies, ensuring the consistent delivery of service.
- The post holder must be able to demonstrate use of excel sheets, update patient records using the Patient Action Tracker system (PAT) and be able to communicate effectively and timely whilst attending the daily handovers/safety brief.
- The post holder will need to be IT proficient and able to access and understand the varying IT systems in order to update and obtain relevant patient information on the appropriate system- The post holder will support the MDT team and senior staff with the daily administration duties, these will include chasing packages of care, equipment and any other relevant information required to support a patients discharge.
- The post holder will be required to follow up on transfers where issues arise within 24 hours.
- The post holder will provide updated information to care homes and patients/ families, & GP surgeries prior to transfer.
- The post holder will also act as an operational assistant providing a professional, confidential, and comprehensive support and overview to the ward Manager.
