We are looking for a new member of staff to join our administrative team, which may be based in either Llanedeyrn Health Centre or Llanrumney Medical Centre, so the applicant must be able to travel to either site. The Patient Care Navigator will be required to undertake various clerical duties associated with patient care and the efficient running of the GP practice, acting as a focal point for signposting and communication between patients, doctors, nurses, other members of the primary health care team and external services. The Practice is paper-light so the individual will be required to have good IT skills to follow electronic workflow processes and good telephony skills.

**Salary**: From £11.44 per hour

Expected hours: 24 per week
