**Health and Wellbeing Partnership Co Ordinator**

**Overview**

This is an operational role, responsible for leading the development of Bromsgrove
Partnership Board / Collaborative key wellbeing initiatives. This is a part time role only.The lead will be tasked with establishing Task and Finish Groups focused on local priorities in order to operationalise the ‘Being Well’ initiative. The role will engage wider wellbeing partners, integrating well
- being and prevention. The function will encompass the following key principles:

- Utilise the collective resources across all sectors to address Bromsgrove Health &
- Wellbeing Inequalities
- Secure a greater understanding of key determinants of poor health/wellbeing outcomes for the people of Bromsgrove.
- Tackle the root causes through prevention & better use of Community Assets

**Governance**

Reporting directly to the Partnership Manager at the District Council and the PCN
Manager and under the direct supervision of its’ members, the Being Well in Bromsgrove
Lead will coordinate and deliver the plan agreed Bromsgrove Partnership
Board/Collaborative. Bromsgrove Partnership Board / Collaborative will report to the
Being Well Strategic Group, Herefordshire and Worcestershire Health and Wellbeing
Board and Worcestershire Executive Committee as appropriate and as requested.

**Duties and responsibilities**

Under the direction of the Bromsgrove Partnership Board/Collaborative (and working directly with partners to build upon the foundations already in place).
- Adopt a role which creates and maintains effective partnerships and relationships with key internal and external stakeholders across the wider health, social care, other statutory and voluntary and community sectors to deliver the Being Well initiative, focusing specifically on Bromsgrove and District
- Lead the development of Being Well in Bromsgrove prevention and well-being approaches that are fully integrated with health and social care and wider.
- Using and linking existing partnership groups and networks Bromsgrove stakeholders
- (e.g., health, housing, voluntary and community sector
- all local services, solutions, and assets) - be a conduit in order to help enable timely and seamless well-being support for people.
- Develop/optimise a range of approaches to engage the community and communicating information about well-being support in the locality, including use of
- Here2Help and other Partnership Board members.
- Develop a Network of Networkers - joining together Social Prescribers, Community
- Connectors and Community Builders (as appropriate) and others - implementing a programme to empower them and other frontline staff to signpost to support available across Bromsgrove and District.

**Communicating and Leading Change**
- Actively engage a range of stakeholders across statutory, third and community sectors, using existing networks such as the wider Bromsgrove Partnership
- Develop a broad understanding of the provision available locally to ensure and encourage the right organisations across all relevant sectors are represented and engaged in Task and Finish Groups to help ensure a joined-up approach.
- Use influencing and negotiation skills to engage partners with differing priorities in order to achieve positive outcomes
- Work closely with all stakeholders to ensure that they are able to respond to changes and align the required transformation within their organisational strategies and processes.
- Manage relationships between organisations which may be sensitive, complex or contentious.
- Effectively communicate a clear vision for Being Well in Bromsgrove, explaining complexities to a wide range of stakeholders and championing change.
- Provide regular reports and make presentations to large audiences on the development and implementation of the programme, presenting complex information in a clear, understandable manner.

**Analytical and Judgement Skills**
- Work within a complex partnership environment to progress the objectives of the programme.
- Monitor and audit success outcomes from projects using analytical systems on a quarterly basis.

**Planning and Organisational Skills**
- Develop plans to achieve multiple programme objectives and oversee the tracking of progress against plans and milestones, ensuring appropriate processes are in place to highlight risks and issues to the District Council Partnership Manager and PCN
- Manager in the first instance.
