**Exciting Opportunity**: Unplanned Care Telephone Triage Clinician

Join us as an Unplanned Care Telephone Triage Clinician for a 12-month secondment or fixed-term opportunity. Conduct telephone assessments, provide evidence-based advice, self-care options, and referrals for patients with diverse clinical conditions.

**The postholder will provide the following duties**:

- Triage and accept referrals direct from all referral sources into the Community Services and sign posting on to other agencies where appropriate.
- Undertake comprehensive risk assessments associated with the care of patients in order to ensure clinicians’ and carers’ safety.
- Act as a role model for other staff and students demonstrating high standards of practice and professional conduct.
- Support the Senior Management Team in ensuring all obligations of service delivery are met.
- To clearly identify urgent and non-urgent referrals and allocate patients to the relevant pathway of care.
- To have continuing responsibility to prioritise referrals through the integrated single point of access (ISPA) using sound clinical knowledge and risk assessment skills to ensure patients are transferred to the most appropriate service to meet their health care needs.
