The Adult Cardiorespiratory Enhanced + Responsive Service (ACERS) are looking for a passionate Tobacco Dependence Treatment (TDT) Advisor to provide specialist support to inpatients at Homerton Healthcare.

Adult Cardiorespiratory Enhanced and Responsive Service (ACERS) is an integrated Cardiorespiratory Team providing care to people living in City and Hackney or with a City and Hackney GP.

This is a newly created post to support the implementation of the NHS Long Term Plan (LTP) for treating tobacco dependence within the Acute Trust. The Tobacco Dependence Treatment (TDT) Advisor will be responsible for the delivery of the new Tobacco Dependence Treatment Service and will implement Trust wide support for adult smokers to quit.

The post-holder will work as part of a team within ACERS, key duties inculde:

- As part of the ACERS team, the post holder will be providing tobacco dependence treatment interventions to adult inpatients and provide seamless onward referral for ongoing tobacco treatment support in the community.
- The post holder will be instrumental in supporting Smokefree Homerton Committee, and work with Information Services for Patient Level Data (PLD) reporting as per the national directives, for inpatient services.
- The Tobacco Dependence Advisor will be responsible for the implementation of the new Tobacco Dependence Treatment (TDT) Service and will implement Trust wide support for smokers to quit.
- The post-holder will be responsible for delivering and teaching evidenced based practice.
