**The End of Life & Palliative Care Assessment Sister (In-Reach) will support the identification of patients with specialist palliative care needs and/or end of life care within the local Acute Trust, assess their needs, and with discussion with the patient, their families and Acute SPC team agree the referral pathway
- Hospice, home, or other care facility.**
The post holder will support the management of patients with complex needs in their home to provide a more efficient, effective, and flexible service that will ensure patients receive the right care by the right person at the right time.
**The post holder will;**
- Work alongside the Acute SPC team on referrals received and identify those who have SPC needs that are appropriate for hospice service. Work with the hospital discharge teams to expediate the discharge of patients home with support or another care facility and follow up those patients at home who are at the end of their life.
- Proactively visit wards (1,2,9,11,12 & 37), departments, and A&E at the acute trust to promote what hospice care can offer to their patients and referral processes.
- Assess new referrals to the hospice using the Responding to Urgency of Need in Palliative Care (RUN-PC) Triage Tool and support the prioritising of patients to increase the access of patients to the hospice and ensure the most appropriate use of hospice services.
- Contribute to decision making and work as part of the MDT to determine the most appropriate intervention and best place of care for the patient.
- The role will allow for supportive discharge from the hospice setting when patients would benefit from intensive follow up to support their symptom management or ongoing monitoring and providing support to Care Homes and link in with community teams undertaking joint visits and handover to primary care and community teams in a timely manner.
- Support the Senior Nurse for Community & Wellbeing in the development of Community and wellbeing including In-Reach
