The opportunity has arisen for a competent and enthusiastic individual to undertake this new role based on CATU, CRCH for 37.5 hours per week. The post holder will facilitate the efficient and timely discharge of patients from hospital and undertake the clerical and administration duties required. The discharge coordinator will work alongside the ward manager and leadership and nursing team to enable seamless and safe discharges and ensure effective communication throughout the multi disciplinary team, care providers, patients and their families.

To work as a member of the multi-disciplinary team to support the discharge process.

Regular communication and direct face to face contact with patients, relatives and carers.

To participate in multi-disciplinary team meetings and actively contribute to the planning of patient discharges.
