Seven Social Care are looking for a Recovery Outreach Worker to work in a mental health service in the Royal Borough of Kensington and Chelsea. This is a temporary contract initially for 3 months with the possibility of placement extension. We are lookingfor someone who can start ASAP.

**The role of a Recovery Outreach Worker**:
The accommodation-based services comprise of 4 properties; you will support adults with complex and often multiple needs. The services include a Hospital Discharge House providing support for individuals that may be homeless or at risk of homelessness upondischarge from hospital. You will provide good quality housing related support to service users with a range of needs in line with client led actions identified in their support plans, and with organisational policies and procedures.

**Key Responsibilities as a Recovery Outreach Worker**:
To deliver housing related support to adults with a range of needs, to understand their strengths and build on these to help clients meet their aspirations in a range of areas of interest and value to the individual.

Improve each client's quality of life; prevent homelessness through promoting or maintaining independent living skills and appropriate move on.

Provide relevant support in accordance with the client's range of needs via support and safety planning and aspirations assessments and through one-to one keywork sessions.

**Applicant Requirements**:

- **Experience**: Some experience working with mental health and substance abuse.
- **Compliance**: DBS on the Update Service, Right to Work in the UK
- **Hours**: 37 hours a week Monday to Friday
