Early Intervention and Resilience Practitioner

To offer targeted intervention to support young people and their families to address the underlying factors that influence health harming behaviour. To intervene early when problems arise to prevent young people’s drug and alcohol misuse becoming a dependency and to deliver a preventative approach in community settings. To address underlying factors contributing to risk taking behaviours and support young people to have improved competencies, problem solving and coping skills.
The role will contribute to the Stockton-on-Tees early intervention partnership approach to build resilience in children and young people.
Under the direction of the Early Intervention and Resilience Specialist Public Health Nurse/Practitioner, the Early Intervention and Resilience Practitioner will work as part of a team and as part of the wider system across Stockton-on-Tees, contributing to the provision of high quality services for children/ young people and their families/carers to address the root cause behind risk taking behaviour at the earliest possible opportunity.
To plan, deliver and evaluate a programme of activity for children, young people and families following assessment by the Early Intervention and Resilience Specialist Public Health Nurse/Practitioner.
To work in partnership with families/carers to build on their skills and knowledge to ensure children, young people and families achieve their optimum life opportunities
To provide a holistic and flexible approach to meeting individuals identified needs
To develop and maintain good and effective working relationships with colleagues and other professional agencies and organisations
To work as a part of the wider 0-19 multi-disciplinary team and contribute to the provision of high quality care for children, young people and families
To contribute to audit and evaluation of all work undertaken
To engage families, including supporting the proactive calls process and engaging families at community events.
To work directly with children/young people and in partnership with families to address the underlying factors contributing to health harming behaviour (e.g. drug and alcohol misuse and sexual risk taking behaviour)
To support the development of skills and knowledge which will empower families to progress, achieve and maintain their health goals.
To plan and prepare suitable resources for the successful delivery of therapeutic intervention.
