An opportunity has arisen in the Performance Management Department for a Performance Team Support Officer to work 35 hours a week.
We are looking for an enthusiastic, motivated person to assist with tracking patients through their clinical pathways to ensure achievement of the 18 week referral to treatment standards and the relevant cancer standards
Knowledge of current cancer pathways and the 18 week referral to treat process is desirable as well as an ability to quickly understand the complexities of patient pathways.

The post holder will support both the MDT Co-ordinators and Tracking Team to manage the target pathway of all patients from referral to treatment or discharge.

- To work with the patient pathway tracking team and clinical colleagues to ensure that patients for treatment are tracked through the ‘patients target lists’ (PTL)/18 week patient pathway.
- To ensure that all mandatory information is collected at source.
- Using the cancer record/Christie PTL solution to monitor all patients through their pathway, liaising with appropriate clinical leads, service leads and other divisions to ensure that all parts of the pathway are delivered in a timely manner to avoid unnecessary waiting for patients.
- To escalate any issues of potential breaches to the Pathway trackers and Performance Manager
- To liaise where necessary with the referring hospital for clarification on individual patients and gather any missing data items.
- To support the MDT co-ordinators for each meeting, thus enabling the smooth running of the MDT meeting.
- To provide support to ensure that the necessary proformas are completed and entered into the relevant electronic systems and the patient case notes.

Additional Requirements

Follow risk management procedures at all times, including the identification, assessment and management of risk within the division.
- To maintain awareness of and abide by all Trust policies and procedures.
- To be responsible for own professional/personal development, including participating in a quarterly appraisal and personal development planning.
- To ensure all the principles and practices associated with Caldicott and Data Protection legislation are rigorously adhered to.
