**Role two**: To cover our Central Locality - including the towns and areas of south Suffolk, Stowmarket and Eye.

This is an exciting time to join our trust, as we undergo a programme of transformation and improvement, delivering high quality care and support to children, young people and their families in the community.

This role will bridge the gap between primary care and secondary care, and the post holder will be required to provide assessment, and brief interventions when appropriate either from the GP surgery or from one of the delivery points across that locality. A consultation and training element will be fundamental to the success of this role and further roll out.

These roles are new and build on the Over 18 PCN Mental Health support which is currently being implemented locally and nationally. Although, this is in its early stages of development and adjustments to this role are made to accommodate the need of our younger client group. The post holder will be involved in shaping and developing the role that we can prove the concept and grow the number of Under 18 MH workers acting as an interface for Primary and Secondary Care.

ROLE SUMMARY

This role will work across the interface between Primary Care and Secondary Mental Health Services as part of an Integrated Model of Care delivered across three localities for Suffolk Children Family and Young People’s service (SCFYP). The post holder will provide support, treatment, advice, education and problem-solving approaches as part of the PCN Teams for children and young people experiencing problems with their mental health needs.

The role will also provide clinical oversight and support to a team of PCN Liaison Practitioners.

BACKGROUND AND CONETXT

PCNs form a key building block of the NHS long-term plan, bringing general practices together to work at scale with other health and social care providers, as part of an integrated system. These networks will be able to provide a wider range of services to the communities they serve and based within said communities.

Most networks are geographically based, and this footprint will act as the hub around which all integrated community-based teams will develop. As they evolve, they will be expected to think about the wider health of their communities, taking a proactive approach to managing population health.

Primary Care Network (PCN) Service for Mental Health has been developed to provide access to mental health services in Primary Care as part of this integrated health and care system.

DUTIES AND RESPONSIBILITIES

The role will work across the interface between Primary Care and Mental Health Services to provide advice & support, assessment, treatment, education and problem-solving approaches to the Local Primary Care Teams.

Service User and Carers
- Initiate, create and maintain therapeutic relationships with patients and carers, ensuring that practice is patient centred and needs led
- The relationship should adopt a ‘partnership approach’ engaging service users in the positive management of their health and wellbeing.
- Ensure that all Service Users/ Carers/ families are given full explanation on their care pathway/treatment options and no decision is taken forward without their consent.
- Ensure co-production with patients/ carers/ families in the development of the care pathway to be used across the PCN.

Clinical
- Practice autonomously in making clinical judgements, identifying care needs and alternative courses of action (as required), managing and providing a high quality of care.
- To be a source of advice on the management of individual cases, including co-morbid physical and mental health needs.
- Facilitate ‘sign posting’ to other appropriate services and occasionally undertake very short-term interventions for clients with mild to moderate mental health difficulties
- Implementation of NICE guidance and other statutory / best practice guidelines (as indicated)
- Undertake joint assessment work (as required) with PCN colleagues.
- Offer advice to or obtain advice for the Primary Care Team on appropriate interventions and ongoing Service User care managemen
