Do you have excellent interpersonal communication skills, knowledge of at least two areas of multiple needs, an understanding of the experience of adults with multiple and complex needs, and an understanding of how multiple disadvantage affects individuals to access services?

We are excited to be offering the opportunity to join our team as Criminal Justice Recovery Service Navigator, in partnership with Birmingham and Solihull Mental Health Foundation Trust’s (BSMHFT) Integrated Offender Health service. This post aims to support individuals facing Multiple Disadvantage within the Criminal Justice system to access appropriate services to reduce the level of re-offending and successfully integrate back into the community.

**The purpose of this role will be to**:
1. Support offenders facing multiple disadvantage into appropriate services and divert them away from prison and re-offending.

2. Support the monitoring and evaluation of the programme.

**To do this we will be asking you to**:

3. Contribute to BVSC Corporate Responsibilities.
- Knowledge of at least two areas of multiple needs (homelessness, addiction, re-offending and mental health) with sound knowledge of services available.
- An understanding of the experience of adults with multiple and complex needs and an understanding of how multiple disadvantage affects individuals to access services.
- Proven track record in building and maintaining relationships with a range of agencies.
- An understanding of Risk Management and Safeguarding procedures when working with vulnerable people.
- Experience of identifying and handling sensitive information and confidential matters in accordance with the current Data Protection Policies.
- Proven track record of caseload management and the ability to develop and maintain comprehensive client records.
- Ability to work collaboratively and constructively within a team setting.
- Excellent interpersonal, communication, active listening and presentation skills, with an ability to relate to, advocate and engage with a wide range of audiences.
- Excellent problem-solving skills, a positive ‘can do’ attitude and a passion to make a difference.

**JOB DESCRIPTION**

In partnership with Birmingham and Solihull Mental Health Foundation Trust’s (BSMHFT)
Integrated Offender Health service, this post aims to support individuals facing Multiple
Disadvantage within the Criminal Justice system access appropriate services to reduce the level of re-offending and successfully integrate back into the community.

**PURPOSE OF THE ROLE**

1. To support offenders facing multiple disadvantage into appropriate services and divert them away from prison and re-offending.

2. To support the monitoring and evaluation of the programme

**DUTIES AND RESPONSIBILITIES**

**1. To support offenders facing multiple disadvantage into appropriate services and divert them away from prison and re-offending.**
- To accept referrals from the Integrated Offender Health Practitioners and other partners based in Custody and the courts.
- To carry out joint home visits and meet individuals in community settings to help them access appropriate services and support.
- To identify needs, refer and accompany individuals to appointments and to offer short term interventions until they are settled and confident with the appropriate support.
- Attend weekly meetings of Integrated Offender Health service to support assessments and review caseloads.
- To work in partnership with external and internal partners, forging positive links and relationships, ensuring the smooth transition of client referrals between agencies.

**2. To support the monitoring and evaluation of the programme**
- Carry out all monitoring and recording requirements required of the post.
- To produce reports as requested by Senior Practitioner and Head of Multiple Disadvantage.
