Trusted Assessor

The post looks to strengthen relationships between our community providers and the acute setting, to ensure that discharges are high quality, person centred and ultimately successful, resulting in fewer failed discharges. The Trusted Assessor will be required to identify and problem-solve upcoming barriers to discharge and demonstrate the abilities to challenge senior professionals where indicated. The Trusted Assessor will be required to work flexibly and collaboratively within multiple MDTs, to proactively identify patients needs and to ensure least restrictive and holistic approaches to care are facilitated. The post holder will be working as part of the patient flow team, leading meetings as required and consolidating information from all professionals, patients and family members to assure a safe and timely discharge.
