Care and Support Co-ordination Team, South Tyneside

About the Role:
The Support Co-ordinator will work closely with the Support Coordination Officer and Support Coordination Manager to provide support with a range of tasks, becoming a valuable interface between operational social care practitioners, hospital discharge teams and service providers. You will be supporting with the implementation of care packages, the brokerage of services and the accurate recording of service and financial information. You will be required to work on a Rota basis between the hours of 8.00am - 8.00pm.

The role will help challenge the status quo and help lead change to ensure Adults within South Tyneside are supported to Live Better Lives.

A key part of our Adult Social Care Strategy, and as a direct response to resident feedback, our new Help to Live at Home Model was developed to support adults to live independently in their own homes and communities. Providing both short
- and longer-term support, including a rapid response service to reduce hospital admissions, the model also provides a night visiting service.

About You:
You will have experience of operational issues within a health and social care environment and be proficient in the use of Databases. With excellent coordination and communication skills you with be able to develop and maintain effective relationships with a wide range of stakeholders.

You will need the confidence and ability to demonstrate sound professional judgements, when making decisions and have the ability to support real change.

About South Tyneside: South Tyneside has a rich heritage and our history has played a major role in regional, national and even world history. The index of Multiple Deprivation 2019 ranks South Tyneside as the 31st most deprived local authority areas in England (out of 317), however there is a lot of variation in the levels of deprivation and affluence across the borough.
